EDIT: Isn't syphilis the one which was probably brought back from the Americas in the 15th century?
EDIT 2: Aaaand I just looked it up and apparently there is also a smallpox trait. Damn my excellent physicians and lack of general experience with the illnesses. Another thing to add to the 'ahistorical as hell' list.
So if the character has the lover's pox, it will become the great pox during the game progress.

if the character has the rakish trait (stress event), it has 20% chance to have lovers pox, 5% for great pox when they visit the brothel.
It can be more deadly then as the disease could cause infections and limited anti-botics then. Not like the super effective cure like we have today with penicillin.

Obviously "great" pox is syphilis. It has always been called like that, great pox or simply pox, napolitan disease etc. though it didn't exist at the time of the game. Or maybe it existed but not by that name.
But the lover pox could also be a syphilis. Like tuberculosis it can take several forms
It adds to the confusion that 'great pox' will often initially present in game as lover's pox, if it is not part of a plague, but RNG giving somebody great pox.
Not sure why they did what they did in game with the diseases, but I believe it is probably supposed to be a play on not understanding diseases and that they often called all sorts of individual diseases by single groups in their 'lack of understanding' of the differences and what caused them.
If you investigate them with edit/debug you can 'cure' both lover's pox and great pox with a command. You cannot cure the 'pre-great pox' lover's pox with a command - UNTIL it becomes great pox. Then it is a removable by command trait.
