If neither the daughter/concubines or wife could get pregnant then you'd assume it's because of the man, rather than the women.
Was that with a faith which allows close family incest?
Okay then this was apparently the ruler's fault. Already spent his ammunition on those 2 daughters, i assume.
Well the ability to take anyone as concubine is definitely a bug, because the interface to "take concubine" does properly filter out characters you also couldn't marry based on your faith's incest laws. Only the direct interaction on a character portrait allows this.
