troop quality is determined by the percentage man at arms vs the percentage of levy's in your army.
as you can have only a limited amount of man at arms (a limit I very much dispise).. your late imperial armies of 100k troops will all be ♥♥♥♥♥ quality.. as you still will at best be only allowed 2 or 3 more man at arms than the lowliest of counts....

This is my instant-spawn army with 6k levies removed to make it elite quality. Lead by Ruler with 1 point in Marshal in my 867 ironman custom ruler game (no mods, ofc) :)
ha! That's awesome. That's on a completely different level from any game I've played. I think the most amazing thing about this is you have something like 62 diplomatic relationships. But really, I have little idea how almost anything I'm seeing in that screenshot is possible. ;)
How do you have 121 knights in your army when you only have 48 knights, yourself? How does that work?

There is a lot going on but to answer your question, mercs or as I like to call them, my private army for centuries. :)
I'll give you a hint as to how to achieve it. Have you ever seen that religion in your game? :D

No, I'm a simple man. I've been too lazy to do too much with religion. I've only used Insular, Catharism, and Catholicism (for organized).
Maybe one day you'll end up with a Ruler that is a prophet with 4000+ Piety and that might change :)
This can't be right because I have zero levies and only men at arms, but still only decent quality. Is this how the game is intended or am I doing something wrong?
IE: I can have a 10k army with 20 champions and 2.5k of them being norse heavy infantry, and the AI can have their derpy armies, the game will consider them both to be tier 2 in quality but my army will often wipe out the enemy with next to no losses, sometimes even wiping out the entire stack, yet they were both tier 2 in quality.
If I were to cheat and add a single 10k prowess knight to the army it would probably be listed as one elite, max quality army.
Troop quality is just a rough estimation, for a better judgement look at the men at arms present in the army and if you're really interested do the math and figure out how much attack and HP the army really has.
Also, I've played the vampire mod back on CK3 release and did test out some crazy powerful knights, raised an "army" of like 5 vampires with very high numbers of prowess and sent them against a massive peasant rebellion to see if it would work, in theory the knights had more attack and defense than the peasants, but it was just 5 people, to my amusement they wiped out the entire peasant army and nobody died, so I don't think there is anything deep or any special mechanics related to the number of troop in CK3, it's just a blob number of atk vs a blob number of defense.
Then if you add certain bonuses to certain troops, build a MAA army of that certain unit and fight against a random army with mostly levies it's no wonder you get to see 5~8k wiping out stacks of over 40k men.
