Yeah had that issue too in one play through, Spain did get partly unified under at least 1 king, he died, it all fell apart, back to islamic control again. Sometime around 1150 I was fighting a muslim horde of nearly 50k troops as the King of Bavaria with less than half that number.
What is the state of Spain in your games generaly?
4/5 games muslims conquer spain bit by bit. Catholics gone in 100 years.
1/5 game I see Asturias survive and see Aquitaine/France move into Catalonia and Valencia.
In my current game Asturias has almost united the entire peninsula and made it catholic. The ruler is still visigothic as well in 1050.

Do I need to personally hold every single county in Iberia? Because France does still own a few little pieces (which vexes me, but what are you going to do?).

no, it can be done from 1066 start. that's how i did it.
yes, you have to have all iberia within your realm. from what i can see you're missing a few counties.
I guess it makes sense, though the achievement description isn't very clear about it. It merely says that all of Iberia needs to be converted to Christian.
Whelp... time to declare war on France, I guess. I keep waiting for the queen to die so I can get them during a succession. She's 60 and obese. Come on!
