Same goes for won fights.
What unskilled said is doubly interesting therefor: If possible you wan't to find that army the enemy Ruler or Heir fights in and hopefully catch him.
If the enemy ruler is not adult easiest way to find him is in his capital ;)
Wage wars for county by county is a really slow way, I want to ask if there is an more efficient method.

You can also always click on the ducal title you want in your kingdom, and have a gander at the claimants list. Perhaps marry one off to your daughter or give them land then press their claim.
Downside is, only works against enemies of lower rank and their realm cant consist of more then 3 counties.
+2 if your are a King and +2 again if you are Emperor.
So even as Emperor you can at max force vasallize a realm with 7 counties but its still a good way to grab a lot of the smaller fries in the mid/late game.
Also Holy Wars like already mentioned above.
One of the scummier ways to accomplish this is abduct a second or third in line heir. Then recruit them to your court and now you can go to war for entire kingdoms. Then if that kingdom is in another empire you can declare war on the empire to claim your dejuire lands.
Don't abduct the first in line. Sometimes it won't let you recruit them because they become ruler. You also have to wait until they're an adult in order to recruit them.
While you're abducting people fabricate claims. You don't have to take one claim at a time, you can keep fabricating and scoop them all up at once when you're ready. But you have to do this before your ruler dies.
Another way is to acquire half the counties you need to form kingdoms. Form the kingdom, then declare war on the person who has the other half.
You can also invite people who all ready hold claims, but this isn't nearly as effective as the above methods.
You can also buy claims with piety. But I think this requires something on the skill trees to let you do it.
Standard CBs:
It's the basic conquest or holy war CBs where you designate either a county, duchy or kingdom to contest and you get that designated area in the peace deal if you win.
There is also a Great Holy War CB which targets a de jure kingdom and goes to war with ANY ruler of a different faith than yours in that kingdom.
Invasion CB:
Tied to the warmonger religious tenet. It allows you a single-use declaration against a target nation for an entire kingdom AND any counties you control outside the area. You can get lots of extra land using this CB by ignoring the target area and taking over control every where else, trying to minimise you war score as to not hit that 100% too fast.
De Jure CB:
Comes in with tech later in the game. Lets you go into a de jure war against someone and taking those parts you are legally entitled too given what titles you hold. They are limited to a county, duchy or kingdom at a time.
Claim CB:
You can fabricate or marry into a claim and take a specific title from an opponent. Note ANY title can be taken this way, even an empire title, and you get any personal holdings the title holder has inside the de jure area of the title as well as any vassals.
