The intro scene completely went over my head, please help

What was this all supposed to show? Was it meant to be understood or do the pieces come together later on? Because I am still as lost as I was when starting the game?

Was this a flashback dream? Who was that sick mother? Why did the surgeon suddenly take care of a girl at the end of the flashback?

It all comes together as you play, there are moments where things fast forward, there are moments where you revisit old memories. I can't really explain without spoiling it for you, but I can say that it doesn't ruin gameplay if you don't know, I didn't quite understand in my first playthrough either.
