Very limited production
I am fairly deep into a game (turn 210) and for the last 30 turns or so my production has decreased to ridiculously low levels. For example, I have a fuel depot II, all upkeep and production costs are paid, it shows I am producing 4840 units of oil. However, I am only actually gaining 12 units per turn. I killed off all of my military units, and turned all other facilities to 25% production to try to see what changes would occur. Now I still show 4840 units produced, but actually only gain 30 units per turn. Am I missing something?

Indeed, the logistics and supply and empire management become burdensome the more territory that I gain (along with the longer that I last).

There so many variables that could be effecting this, plus I'm not skilled or knowledgeable enough to point out some more likely culprits. Nevertheless, I'll add a few ideas from repeated experiences.

First, you must have either luck (the right science discoveries) for metal and oil (or mega-scaveging) or a higher percentage start (over 110% metal composition) for you map. Metal shortages cause huge backlogs for building projects (which in turn create pop unrest).

Second, you must develop QOL buildings to bolster pop happiness. I get too overloaded on developing IPs, roads, RRs, and mines only to suffer pop strangulation.

Third, learn what perks each political system gains you, especially in strategy card turn draws. Dem and Com are great in some ways but not all. Having lots of cash without having the right science technologies makes for more frustrating games (TME).

Fourth, choose the easier logistics for setting up your game. i've found the new 1.25 updates to allow me nearly twice the time before infrastructure implosion.

Fuel depots don't produce oil, they store it. You need an oil drill for production. Unless you meant the fuel depot isn't increasing the max available storage? In any case you can left click on the asset (either from the map or the MNG menu) and check the asset log which will tell you what's going on.

I think you need to produce more fuel. I think you are fielding a large army without producing enough fuel to go around. Check the fuel consumption per turn for the units in you army. If you have a large surplus of fuel, it might be an issue with the logistics. You can try to use custom pull points to temporary get the fuel to where it is needed.

Check graphs, oil supply. It will tell you if production/use is the issue (5k production isn't a lot at this stage).

Check bottlenecks view, if there's black then it may not have enough points to be delivered and is piling up into a zone, waiting to be delivered.

The fuel depot is storage space, not production. You generally have limited storage space for food, fuel, industrial points, ammunition and energy. Your storage space for those resources depends on the population number of your cities. If you need additional storage space for whatever reason, you build depots or in the case of energy power banks. They're generally low-priority buildings in early game because your economy starts very weak and the available storage space is more than sufficient. Resources that exceed the available storage space are not lost, they're automatically sold on the market. Only if their market price falls very low (I think below 0.2 CR per unit), they're simply thrown away. Happens with water on an ocean planet or with fuel on a jungle planet that has oil deposits everywhere. Metals, rare metals, radioactives, machines and high-tech goods can be stored in unlimited quantities, they don't need storage space at all.

For the fuel production, you build oil drills on fuel deposits, recycling facilities on ruins, or biofuel factories that convert food into fuel in your cities. Higher technology levels offer you more options.

Player cities generally start on a ruin hex and have a private scavenging facility on that hex. Nationalize that scavenging facility on turn 1 for 500 CR and it turns into a recycling facility, providing you with a small but reliable early income of fuel, metals and rare metals. Those resources are the foundation of your future economy. Your first construction project should usually be a lvl 1 bureaucratic building, to get your administration and research going. After that, you probably need to build an energy source. Researching solar panels first is advisable, provided the sun is close and powerful enough to not make them useless. Solar panels only require workers for operation, means they only consume food and money. The normal power plant burns a large amount of fuel per turn, which has to be provided in one way or another.

Thanks for all of the help. I think the issue was my mistaken belief that the fuel depot was producing fuel as opposed to storing it. I should have known that! Anyway, thanks for pointing it out.

Gotta be careful when mechanizing your army, especially if you're investing models with large engines and heavy armor + high caliber guns. Mech infantry formations, especially custom ones with lots of towed weapons and infantry can easily put you in the hole if you don't budget your oil production accordingly.
