I want to ask about new game+ if I start it my whole map will be clear or it will stay the same way I finish it?

I'm not sure I understand exactly what you mean.
New Game+ means you start a new game. You'll do the entrie story and side quests and all of that again.
You just keep your character level, your skills and your equipment as it was when you finished the last quest in the main story line.
Plus, you can get improved versions of the top-tier weapons and outfits.

I know that all items except Shadow stalwart will stay in your inventory but i want to know about map if i want to have 100% do i need to do it in NG+ or i can do it in first playthrough

You mean for the achievement?
Afaik that only refers to the DLC. And If I remember correctly I did it in first playthrough. You just need to clear all quests and errands in one playthrough.
