Well actually normal, hard, very hard and ultra hard.
But if you skip these and just do ultra hard you get 'em all. :D
I always hate difficulty sliders which makes the players do less damage and the enemies more. To me it's sign of a bad RPG design. Although if you look at it, Ultra Hard is supposed to be a NG+ thing since it reduces your damage output by 70% and increases enemies' to 200%, which makes sense since you will be super buffed coming to a new game with all the perks and buffs.
With how bad it was to aim with a controller, and this games sure wants you to make split second shots with your bow(s), I decided to go with Hard Difficulty(Player 90% dmg, Enemy: 125%).
I am enjoying my game now instead of every battle being a tense encounter. As always it's upto you how you want to play it.
so i recommend playing your first play through on ultra hard and enjoying it and taking it slow and learning the game through that difficultly.
since you did say you only do 1 play through, you can finish this game incredibly quick just rushing the main quest so yeah, try it on ultra hard first play through.
p.s. I tell you what though, i didn't do the dlc on non ng+ and it's pretty ♥♥♥♥♥♥♥ hard on ultra & ng+ but fun and challenging. nothing really feels like a cheap death in this game, you can manage pretty much everything. the only thing that WILL be a problem will be resources later on in the game and you may need to farm a little bit sadly. specifically blaze as a lot of the later enemies stop dropping it.
Normaly i pick the hardest difficult and take the achievement as a bonus, but at this game it seems not possible and like spectre wullf was saying ultrahard seems like a mode made for ng+
