Does the doctor need his canesword?
Does he have a health issue or is he just using the canesword just to hide a weapon, could his last name be House?

I assume he just disguised a sword and nothing else. Walking around with a weapon out in the open looks suspicious no matter what, even if we assume it's legal to carry a weapon then it would still draw attention. As for why he needed a weapon on him at all times, that's certainly a matter of debate, be it if he already had a lot of enemies to defend himself from before the abduction or if he was doing something on the regular that could involve killing people.

The doctor looks like he's from the upper class (the New Blood store showed pictures of him several times), so I think the cane is a status symbol, as was common at that time. The sword merely links this fact to cool gameplay.

I think the most likely answer is that he has it for self defense, considering the fact that people like the huntsmen are around who go around killing people and robbing them. And I really don't feel like the doctor would be doing stuff that has him killing people, judging by his poor trigger discipline and seemingly lack of proficient skill with his own sword, he probably hasn't seen much combat.
I'm hoping for a difficulty setting where the doctor phonk walks and loudly bangs his cane on the ground.

Changes all enemies to melee and compels you to duel them all.

While self defense might very well be the reason, the impression I got is that the appearance of huntsmen is a relatively new phenomenon and is localized to the Gloomwood area, I'm not sure how much Doc knew about what he was getting into when arriving.

I disagree. The Slaughtered Goat interior implies the long term existence of the huntsmen and 'monsters.' See the ads on the noticeboard by citizens requesting huntsmen services. I reckon self-defence was always necessary in a dangerous place that has recently gotten more dangerous due to the Countess' activities.

Plus there could be more groups like the huntsmen that could pose a threat to average people like the doctor.
