//SPOILER (The Basil Theory)
Reminder this is just a fun theory. I don't actually believe this as canon.

The entire game is from Basil's perspective. He can't believe that Sunny would [do what he did] and he can't believe that he helped. As a coping mechanism, he created <SOMETHING> in his head to be a scapegoat that he can blame for both his, and Sunny's actions. As the game continues, Basil is imagining what Sunny must be going through (the gameplay) and is imagining reasons as to why he never left his house. He is also imagining excuses as to why Sunny din't support his friends after Mari died. Essentially the entire world the game takes place in is HEADSPACE, so the actual HEADSPACE is a HEADSPACE inside of a HEADSPACE. Basil imagines that Sunny is going through the same thing he is going through (ie creating a HEADSPACE). It would explain why both Sunny & Basil can see and interact with <SOMETHING>. Sunny's dissociation and identity crisis is real, but we're seeing it from Basil's perspective.

There are some things I don't really get about this theory, such as why would Basil choose to go missing? Sunny/Omori obviously have a large amount of control over Headspace, so why wouldn't Basil? Or does he have no real free will in his Headspace, due to it being Sunny's perspective?
Also, how would the Basil/Sunny fight happen if it's just Basil's imagination? Is he imagining Sunny fighting him, or is it like the real-world sections within the game?

I feel that (in the case of this theory) Basil going missing happens because Basil sees Sunny moving as Sunny abandoning him. I feel that Basil does have control over headspace, but he's imagining what Sunny would do, so he's trying to replicate Sunny's thought processes. I feel that the fight scene DOES actually happen, but the one we see isn't reality. Basil is imagining this section in the hospital bed after it already happened; this time, however, he implemented <SOMETHING> as keeping Sunny from being able to do anything but fight him. This is why the shading in the hospital animated scenes is very different. (even though I know the shading is different because it's in the hospital, as I said, just a fun theory :])
