Note that I haven’t finished the game yet
If you want to test the waters, you could check a playthrough until the point a door is knocked on and (optionally) opened by the player in the real world at night time after waking up the first time (the first part in the dream world is essentially a tutorial anyway until you wake up). If a scare of that level is tolerable, you'll be fine.
Either way, no full blown screamers, fortunately. And be prepared for creepiness if you (optionally) check the real world mirrors or interact with black holes/stains from Keys. I find a lot of the creepy stuff is optional to engage, you could give it a wide berth or minimally interact with it if it's too uncomfortable.
Edit: Never found BoI or Undertale overly horrifying (though photorealistic youknowwho was a sucker punch, didn't appreciate that in a cutesy RPG). It's definitely a step up in scary from anything found those games, given it has some rare few minor jumpscares.
And considering the subjects it handles... Well, expect plenty of creepiness in certain moments.
Also any reviews that might say anything about "mental damage" are likely not referring to the horror aspect, but to the heavy subject matter. It touches on anxiety, depression and suicide. If you are not in a place to handle some of those topics they can really cause some damage.
TL;DR the Horror aspect isn't too scary, but the heavy mental topics are what you need to be prepared for.
but they don't make you scream
they're more like: "Oh i didn't expect that" and then you ask yourself what that thing even was that you saw
The game is much more about the psychological horror aspect. Specifically, the content warning it has. If you are or think may be suffering with depression, anxiety or suicidal thoughts i currently cannot recommend playing this game it will probably make you a lot worse, it won't make you any better at least. Speaking from personal experience. Games like "Celeste" or "missed messages." (by Angela He) helped me. But this game will not do that.
If you aren't bothered about stuff like that then yeah i recommend it. It's very well made. It feels very human. And it's fun and lasts you about 20hrs. Music and the art is very good too.
Hope that helps answer your question.
