There's actually a massive difference between the two. Seriously, adult content means anything not suitable for younger people. This has been the case for literally decades. This include drug use, alcohol use, foul and pervasive language, etc, etc.

Also, you do know the sexual Content tag is user added, right?
All those tags are user added.

Why don't people know this?

The tags for games like this are literally almost always going to have troll tags in them.
One of the Alchemist of Arland games suffered from a Sexual content tag for no reason.
