ITEM ATTACHMENT ISSUES
Im getting an issue where my attachments are un-attaching themselves (or deleting, or attaching to things I didn't put them on; my axe has my helmet attachment, my pick removed its attachment and refuses to accept another one, my knife has a sniper scope). I didn't attach them this way. Its like the game is confused about what is where

I've been using attachments for things since prehistoric times and now too, but I've never had anything like this before. Are you sure you don't use mods?

I play vanilla - 2 night ago - in the middle of my base - not using ANY benches in the moment - I opened up my tackle box and the attachment on my active knife was inside (the tackle box shouldn't allow it to be in there period). Looking at my knife, it appears that the attachment is there until I try to move it, then the mod spot blanks out. I took a screen shot. Now, if it try to add the mod back to the item, such as my knife, the moment I put it into the alteration bench, the box I would put the mod into disappears along with the rest of that line - like the item refuses. I don't know how to share my screenshot that includes the tacklebox having the mod and my knife in the quickbar still having it. It has the same title as this thread.

I would like to add that when I add alterations I put in the item from my inventory (not the quickbar), I put in the attachment and watch it finish (not closing the window), I pick up the item and put it back to my backpack before moving it to my quickbar. I started doing this after my previous bout of things moving around, and my attachment mods showing up in other containers. I break both items back to parts, rebuild both knife and knife attachment incase one of the the particular items is corrupted.

My medical pouch also recently dumped all its items on the floor and became slotless. I don't remember exactly what I was doing in that moment.
