Pistol Start - Fast Monsters?

I want to try a pistol start / fast monsters run. How does one do this? I don't see it in the options.

For fast monsters, I think what you can do is follow this guide: https://steamcommunity.com/sharedfiles/filedetails/?id=2027379157
But instead of using "-skipmovies" use "-fast" instead.

Then for pistol starts there might be an easier way. But if you restart a level it will revert to losing all your weapons and pistol starting.
