Other possible locations not mentioned yet:
There's a screen to the right of the spider castle with skulls and octoroks, exiting through the bottom of that one has the gear sometimes.
Also if you exit north through the place with the big purple boss (just north of spawn) and continue out the top, then go west there is a lava peninsula that sometimes has the gear.
(1) I gave up on finding it in my first run-- which was otherwise completely done-- and used a terminal code that gives you 10 meat, but resets ALL of your progress. Still, this way I figured I could spam as many trips to the overworld as necessary without waiting for meat to generate (or having to farm it). After an hour of wandering around with a map and essentially unlimited time, checking every chest location (because the gear is supposed to replace a chest location-- as every described location in this post does) . Couldn't find it. Gave up completely.
(2) I selected "reset game" from the start menu, which reset all progress and gave me a new map layout.
(3) I spent 3 hours this morning grinding through the early game so I could do a reasonable job of exploring the overworld-- and thanks to the tips here, and the guide, and the map, I found the gear in a chest location, "right of the spider castle with skulls and octoroks, exiting through the bottom of that one". Great! I went and played something else for a while, and then. . .
(4) When I reloaded, I had all my original progress from the original run back, with the original layout, as if the game reset never happened. I figured, okay, it must be that after you enter that "cheat code", no progress is saved after that, not even a total game reset. But now I was back to the original layout where, I swear, I've checked every location 10 times and the thing just isn't here.
(5) Now, if I enter the cheat code again, the overworld map is the one from the SECOND run, AFTER the game reset, where I was able to easily find the thing. But that's no help at all, because every time I reload the game, I'm back in the FIRST run, with the layout where I can't find the thing anywhere.
So. . . I think. . . something is wrong here?

The strange behavior of resetting the game after you use a cheat code is a legit bug, I think. But I doubt it would affect many people.
