It's both.
The main game is a prequel, but there are scenes set after the events from the first game. Especially near the end. It's highly recommended to play Tormented Fathers first, as many events from the first game will be spoilered.
No, you won't understand the story at all if you haven't played part 1. This acts like both a prequel as well as a sequel, and as such you have to be familiar with part 1.
