Report a translation problem
The 'System Status' map at the terminals will show "Unresponsive" systems count, which I believe is associated with an anchor that has been opened up but not had all the puzzles completed to activate it. So if there happen to be any Unresponsive ones then that might indicate a missed Anchor. Though as you may have confirmed, the red boxes on the map with empty/hollow white on the inside are unresponsive/incomplete ones. Where filled in red is activated.
I can't remember if there are any hidden rooms that might be related to that status without seeing an incomplete one on the map.
I'm not sure if the achievement requires also doing all the puzzles to get all corrupted message logs. I'm not sure if I can confirm much but from a save file I might be able to see something, but that may need to be done on their discord as the file is binary and not text. If no one confirms/responds, just let me know here if you want me to try to take a look at the save.
I don't really understand what you mean by looking at my save file. Do you want me to send the file to you via discord or steam direct message or something?
Yes, something like that was the thought. I wanted to see upfront if that would be something you'd be willing to try, and if you had a suggestion on what way might be preferable.
Regarding my thoughts on the save file:
- To check if an anchor or something isn't marked as completed/collected for that specific save, since as you mentioned it may be rather tedious to recheck/solve them all manually just to see if something showed as missed despite the Status Map and other achievements. (Misleading map/bug or solved in one save and got achievement but then not all completed in current save, etc; depending on how it's checked.)
- Also, to consider things like seeing if we could mark one as unsolved so you could then solve it and see if it triggered the achievement (in case something odd happened and it did not get recorded the first time). Though some might be re-checked on start now.
Feel free to temporarily add me as a friend on Steam and we can work something out if you want to try that. (Else you might find me idling in the user list on their discord, though I don't have a registered account (I generally avoid discord) but for this it may be the simplest way to work on that.) Mostly, plan would be to take a look and see if I can find something that stands out or something to test, then let you know if anything fruitful comes out of it.I should clarify that I'm not one of the devs but thought I might be able to help if you got no initial response or no one else seemed to report having the same issue or realize something overlooked. I've been clarifying that in past comments but managed to forget in my response this time.
Originally dev/publishing was directing people towards their official discord server (their invite[discordapp.com]), so their discord might be a more direct way to get in touch with them or feedback from any community that might be active there. (There are bug reports and questions channels)
Side note: a very quick search on discord showed someone wondering if they did not get the achievement due to using the hints system, but it was only a passing comment so please do not assume that is the case yet as I don't see it confirmed anywhere.

But for anyone else that runs into something like this and finds this post -
First- indeed, the Hints System being enabled and used will prevent the "No Anchor Too Great" Achievement from being possible for the specific save file it was enabled for.
- While checking on this, there is a warning message that will show in the settings when the Hints Enable setting is highlighted. If you're not sure about the Hints System, it was introduced later on as a feature after release. (intended behavior)
- If it was never enabled for the save then the next best thing is double checking the System Map at a Terminal to be sure there aren't any remaining items.
If it's been enabled just to see what it is like and this is a concern later on when finishing the game and all puzzles then it would be good to restart a new/second save early on and not confuse which it may have been enabled for. But it's a good chance to replay again in the future when the puzzles seem new/fresh again. :)
