Having trouble with the toilet flushing achievement (possible spoilers)

I'm just finishing up my second playthrough and I made quite sure to flush every toilet this time, but the achievement still won't pop. I had previously gone back into my original save and flushed every toilet I could find, but that didn't work either, so I figured it might have to be in the initial "run of play." But now I'm convinced that I have to somehow be missing a toilet somewhere in the game.

Does anyone know exactly how many there are? I'm pretty sure there are six. The first chapter doesn't have one, and then the one where (spoiler) you move back home doesn't seem to have one either, even though there is a bathroom in it. Am I missing something here, or is the achievement just bugged?

There are actually two toilets in the final level, one on the lower level and one on the upper level of the house. With these two, there should be seven toilets in total you need to flush in order to get the achievement.
