Because you can show all the gore in the world in a videogame but start getting potentially offensive to certain people's belief systems (it's not even like this game sets out to actively piss on the religion, it's mostly just a incredibly dark reinterpretation of a common religious tale) and Nintendo will have none of it.
Ninty's censorship policies are nowhere near as strict as they were in the NES days but they are still undoubtedly there.
1. Religious themes
2. Disturbing factor (huge example: listen to the credits music after ending 10)
3. Items and monsters that don't bother to fly under the radar (Mom's Bra/Pad, the Vis mobs)
All in all while this game is ultra addicting, I would not want my 4-9 year-old relatives playing this.
It's a good game, but I'm not shocked it doesn't fit Nintendo's family-friendly branding.
Like the "Tough Love" item that looks like brass knuckles and causes Isac to spit teeth
Or the item that looks like a can of dog-food that's called "Dinner"
Or the item that looks like rotten milk that's called "Breakfast"
And "Mom's Purse" which is full of pills.
And the "Best Friend" item that's actually an exploding corpse
I thought the same but according to the numerous posts here I guess Nintendo really is only interested in family friendly puffta games (and their quick buck shovleware of course). Guess I have no idea what all the hype was about bringing "mature" titles to their system(s) and showing off gore from different games was about? It just screamed, “Look at us we got hardcore games too!” to me…. But maybe it’s just as someone said earlier you can have all the gore in the world but start getting potentially offensive to certain belief systems and you get scrapped.
It's main character is a naked weeping child who fled to a hellish basement to escape from his mother who has abused and neglected him and tried to murder him because god told her so. It continually references child abuse and humiliation. It's a very compelling game but very dark and very disturbing. I'd be surprised if sony or microsoft would allow it.

Just because a game's not grimdark and edgy enough it automatically becomes childish?

You also know that NIntendo doesn't actually make all the games that go on their consoles right?
Sonogamesy isn't responsible for many games on their systems, that's for sure.
Same goes for Micro$oft.
The so called "quick buck shovelware" can be found on all platforms.
