???'s Soul achievement for completing the game with Cain?

So I just beat Blue Baby with cain for the very first time, and once I beat him it said "Cain's Eye has appeared in the Basement." Everything was going normally until I re-started the game to get my achievement but it said (in the achievement box) I unlocked ???'s soul instead of Cain's eye. Is this normal?

Yes, it's a known bug. Most of the chest achievements, secrets and item unlocks are switched around.
