Take whichever comes first. If you've had very good luck with it and find the other, stick with what you have. If not, then switch them up.

moms pearl. Just to note, soul hearts are way better to have, If you get them you can not grab them until you make a path to the boss room, and then stock up on em to get a better chance for a deal with devil/angel. If you take red heart damage you have severly reduced chance so every soul heart makes a huge difference. Even after you can't get a deal it still makes for good overhealth unless you get polariod, at which point you get the polaroid.

Mom's pearl giving %10 chance of soul heart drop it's not a big deal, pinky eye is much way better with that poison damage, you can kill enemies so easly with that.

While Mom's Pearl is not bad, it's not that useful most of the time. It can help, that is for certain. However, I would pick Pinky Eye in nearly all situations. If you're low on health, super doubtful the pearl will save you. If you've got plenty of health, then it's not going to make any difference in your run.
