When I play as Cain and I use a Luck Up pill it feels like my luck goes way down. Is this accurate or am I just imagining it?

First of all. Cain's starting item does not directly affect or change the actual luck stat but just makes the chance of a drop higher.

Secondly, luck is a hidden stat that starts at one no matter what character you use and can only be altered by luck up and luck down pills. Luck does not change how likely you are to get good items, it only makes a drop after a room more likely and increases the chance of shooting a tooth after picking up the item tooth shot.

And Finally, the answer you were looking for. You were just imagining it. Positive luck just increases the chance of a drop nothing else. The same however can also be said for negative luck whihc is glitchy. Due to some error having -1 luck gives you a higher chance of receiving a drop after a room than positive luck would. Infact at -2 luck your chance of recieving a drop is a whopping 97.5%

Now heres the part where lucky foot comes in. With lucky foot and positive luck you will increase the chance of drops to a percentage I read somewhere but have forgetten. It's pretty high. However with negative luck and lucky foot you severly drop the chance of obtaining a card, trinket, or pill, though I believe any other drop will still have a rather high chance to spawn.

So the only way you could have became unluckier is if you picked up negative luck pills which is impossible with cain as his lucky foot will stop negative pills spawning and instead spawn the positive variant.

Well I had the left hand and before I picked up the luck up pill I was getting a few good items from the red chests and quite a few soul hearts. However after using the luckup pill all I would find in the red chests were troll bombs and spiders. Perhaps I was just having some bad luck, thanks for the answer :D

Luck affects 2 things in the game.

1. Chance of item dropping after clearing room
2. Chance of shooting teeth with "Tough Love"

Lucky toe and Lucky foot does not affect luck stat

Negative luck makes drops almost guarantees a drop after a room is cleared. If you take 2 luck downs, it's a huge positive. It does have an interaction with the lucky foot that something bad, however.

It may interact with the luck stat but as previously stated the lucky foot/toe does not directly influence or affect luck.
