It's all randomly generated, but the item itself is unlocked from the start, if that's what your asking. It will always be in the item pool, I'm pretty sure. Just play a bunch.

You need to have the wrath of the lamb expansion bought and downloaded before your can get polyphemus

oh, ok... Thanks "thatguyinthecorner" I've played a fair amount, some offline and have never come across it. I am going to get wrath of the lamb if rebirth doesn't come out soon =)
