i smell bull in this post
There are tons of unique items and many of them are unlocked via achievements (600+ at the time of writing). Most exotic items also combo with other exotic items, so you can get some pretty wacky item combos. There is also mod support, so there is even more replay value.
Probably the only issues I have with the game is that it can be difficult to see what's happening in some of the more "bullet hell" type moments. Some items/secrets in the game are also very cryptic, to the point where you will inevitably at some point need to use a wiki just to figure out something (though some would argue that is what makes rogue-likes/lites good).
Outside of those, I would say it's still currently the best of the rogue-lite genre.
Yeah for people that already beat all the old content... Its nothing but beast and corpse runs back to back >.>
