Any location indicated with a question mark is a place you haven't visited yet. However, each system in the game is split into 3-5 regions or so. Sounds like you've been getting the achievement for being in (or simply traveling through) every region of whatever system(s) you were in.
Each system has it's own little set of challenges/local achievements that you can use to unlock extra-fast travel in that particular system. When viewing your Inventory or whatever, look for the 'Data' tab on the same menu bar then you can view your progress under 'Challenges´'.
They probably should have moved getting that perk much earlier into the game. You're effectively crippling completionist playstyles right out of the gate. The amount of forum threads about this issue are many.
Good point. I feel similar about locking extra-fast map travel behind challenges that also have to be completed in a certain order.
IMO it's actually more fun exploring and finding things prior to having this perk as it removes a bit of the mystery.
