May not mean its free of "all" loot - ill let devs speak for that.
What I already found out is:
* White frame with question mark: not visited yet
* Yellow frame: main side mission marked
* Blue frame: side mission marked
* Red frame: marked as target destination by player
* White frame: no mission or not marked by player
* White frame with a check mark: you visited the site before and you concluded the special challenge
* Green dot or check mark: you are currently at this location
* White dot: you visited the site before (and if a challenge exists, you didn't concluded it)
White dot or white frame with check mark both don't tell you if you found all important stuff there (like Aeterna Bypasses). This is the basic problem. I cannot tell of it is worth looking around again at those sites or not. It seems that the devs forgot to implement a marker that will give you a hint to this. Or they are planning to implement something else like a perk of a new (not known yet) companion to help you with that later in the gameplay.

Where can I find info on this, google returns basically this question. So, I see there's greyed out check marks and greyed out check marks with a white chevron below them. I cant figure out what the difference is.
I have the same question.
The check mark does NOT mean you have 100% the area, you will be notified while in the location when you have 100% an area, but nothing shows on the map that this has occurred. Later in the game you will get a companion perk that tells you the completion percentage of all locations on the map. You can go back to those locations that you have not 100% completed to complete them at that time. There's no reason to know this prior to that point because you can't possibly 100% complete the game until after you have acquired that companion.
An upwards facing Chevron indicates the mobs there are above your level. A downwards facing Chevron indicates the mobs are below your level.
