If I had to guess, I would say it seems like any occupied building that takes any kind of hit has a chance to hurt or kill an occupant even if it doesnt make sense. I have also been able to target and kill occupants if I had LOS too, so it is kind of a reasonable setup, but they also added the "if you miss but hit the building stuff can take damage" to keep it from being a total slog...the fact that ANY weapon can trigger that second bit (seemingly), makes it pointless, because the occupants are limited to where they can shoot out, but not where they can take damage from.
I may test it more.
Also, it is nice that fortifying makes it so you cant be assaulted. That helps a little bit.
I find that using heavy infanty squads in conjunction with buildings (that haven't been demolished) can be effective. For example, spawning in a squad DP-28, then having a rifleman and one of the DP gunners garrison the house while the rest dig foxholes around it. That way, the inside isn't crowded and the DP gunner can take up empty windows where the enemies are coming from.
The foxholes around a bunker are a neat tactical idea,
the game offers so much play finesse and tactical details.
