I used to DM for an online game, and also became a major coder for "fixing" things in the game. I still swear that players would lay awake at night, dreaming up new ways to cheat. After I would stop a way of cheating or abusing the system, usually within a week somebody had found a way around my fix. It's a never-ending battle.
2. Can't. Then people with an honestly crappy internet connection would not be able to participate.
3. Again, can't. From my own coding experience, I can tell you that it doesn't work. If you ban a CD/game key, it takes them 5 minutes to find another free key on the internet. If you ban an IP, they'll just use a VPN. Also, many modems and routers these days use a different IP every time you open your internet browser just as a general security feature.
Probably 90% of my 'Risk' time is in Solo Mode. Just me versus 5 AI players on the "hard" setting. I find it relaxing.
