Okay I'll ignore the achievements! However, even with your advice, I can't figure out how to get the missing skins. Perhaps it should be more clear, or perhaps it's my error.
An example: Reaper has two skins that appear to be his female alter ego. They're black and shadowed out because I haven't unlocked them. I have beaten all the challenges in default difficulty, and I've played custom difficulty and beaten it with everything turned on at least once. What else do I need to do to get these missing skins? Thank you!

Sure! It's definitely possible to win at the higher levels consistency. I kept playing and ended up being able to do it with most combinations of party members, but at first I was only able to manage it with certain characters. Reaper in the middle row, and either blue card using mage in the back row. Filled my deck with reaper's doll cards, used them with the mage in the back row to charge her abilities. The dolls are way better than defense cards, they can block a whole hit for free and if you time it right get a hit in as well.
In general, offense over defense. I almost never use green defense cards except early on. When I don't win with reaper's powerful dolls, it's usually because I'm using a character who can get >20 damage with their ultimate ability, then giving them all the >20 damage trinkets (there are three) and the one that lets them fire it off at the start. Merlin/Melisandre gets these free ultimate ability cards that I always stick "guaranteed draw" on, then duplicate. Mr Anthony/sexy girl comes with cards that stun the enemy, which are incredibly strong. Use them just before the enemy bar fills up, even if you only have a partially charged bar, and it resets all the enemy bars. Reaper has a two cost skill that gives him max charges when anything dies, including his dolls, so he's a great choice for MAX ability spamming as well.
Other than that I've won because I focused on getting brocky > two hundred hp, or because I finally got good at using Sancho/Sancha's skill set. The archer, don quixote, and the assassin aren't as straightforward overpowering as the mages and are more challenging to me. Good luck!
Edit: forgot to mention merlin/melisandre's "absorb one hit for free" orbs, those are also amazing and better than defense cards! You just need to survive long enough to kill everyone with upgraded max abilities!
Instead of getting equipment, consider just getting experience points from chests and rushing the best abilities and max upgrades. Then fill your deck with only colored cards, and rely on using maxes a whole bunch. That means get rid of all the grey/green, usually. Don't be afraid to not take a card and take a food instead, I usually pass up on 3/4 cards and only take my favorites, unless I see a whole bunch of favs in a row. Try to carry two-three food at all times when stepping on events!
What I remember about pirate chick is beating her by out dpsing her, not worrying about defense. You need a high enough dps to break her barrels quickly and get back to hurting her directly. You can't really do too much to protect yourself from her instant-jump-on-you attack, so again, dps rush! Archer to me feels like a slightly weaker version of the blue card mages, but she still works. Just get whole bunches of yellow cards, remove grey and green, and upgrade her max asap. Don't be afraid to fully charge and expend her cards since there are ways to get them back, and most fights, including boss fights, will be over before you shuffle more than once or twice anyway.
Explorer has never worked for me as a main damage dealer. It's cool that he can charge up from grey cards, which makes him an awesome support/off tank for characters that have good grey cards like sir anthony. Although I'll still remove the gray cards I start with that give me fatigue. Just like your starting green cards, they're useful for preventing damage early on, but slow down your dps later.
I hope we'll get 3 person parties one day, but we don't at the moment it seems.
Familiarize yourself with all your experience point options, become aware of whether each piece of gear is better or worse than what you can spend the points on! I always rush good max level ups too, like for merlin.
Also don't forget to skip cards for food. You can play the whole game without food and still win, but if you smartly use food you can get a lot from events and it really adds up.

I'm guessing one can be obtained from the event that want u discard 16 value cards of it?

But parasite carpace I HAVE no idea, the only ''?'' i can think off is that 16+ cost event , but I had it like twice durning first 5 runs, now ive finished this game over 40 times + many restarts after some chapters just to check options on diffrent ''?''.
looking at the % of ppl who got it (at least on playstation) more people got that than damned sword (that u can get from shop or a reward from those 3 random quests at the beggining of each map) so it can't be that well hiden, mayby im just hyper unlucky or somthing :/
