Has anyone gotten all achievements?

If so, what were the most difficult ones, and do you have any advice

I've started on playing this a few days ago, but I think the most difficult ones is the God of War Achievement and it's derivatives. God of war is basically win vs an enemy with 3x more power compared to yours and other guides mention that the best way for it is to force a defensive battle and outlast your enemy (via blocks/heals/negates).

Other than that, everything else is pretty easy, if not tedious. Just pick a country with good economy ideas (or self improving economy like wood elves) and recruit all unique heroes and do their quests (also search for the hidden heroes and do those too).

After that, then it's all about finishing every country once.

No, defense is a bad way. The best way I got this achievement is to get attacked by an enemy and run away from him until the timer runs out.

It's pretty easy if you luck out with an all melee lineup from the enemy. Lifesteal charas make it easy. Can't recommend it with anything ranged though. In my case, if you bought the DLC, they have that one character in Yan Kingdom that can decimate armies by herself. And since she's on horseback. Running around the field is pretty easy. Just give her anti ranged gear and regen equipment
