That one achievement that nobody got (yet) [spoilers]

Checking the global stats for achievements, I noticed that there's one achievement that nobody got yet:

Mad King.

I'm a bit too burnt-out to go for a second playthrough, never mind one where I play a psychopath intentionally making life miserable for people. I wonder though... what would the requirements for the achievement be like?

Maybe have a bad ending for all family? Her slide is grey if she runs off on her own or you marry her to Varys.

It clearly states that you go mad but i did not get an achievement. Not ever Worst Parent / Husband of the Year. Meh!
