Going purely based off of visual appearance there is no simple way to tell that Kaine has male genitalia in her base skin.
In the book there are short profiles on in-game characters containing their general characteristics: gender, age, height, eye color etc. In there Kaine is marked as "Hermaphrodite". And in the interview Yoko Taro explains why they decided to do it. (long story short: it was his intention to do this with Kaine in a way that the player will never be 100% sure about it, so you are proof that he succeeded, really)
It didn't had to be explicitely stated, which is the correct way to handle gender issue, but their past story would make you understand that she was ostracized long before merging with a shade. Except that her swwet grandma brought stability in Kainé's life and raised them with all the support and love a parent can have.
While there was an irregularity in the cloning/birth, Kainé does seems to consider themselves a full woman. Anyone who say otherwise can... well, Kainé can very well tell you just how much $#*% to eat.
She only became a shadow long after she was banished from her village and the whole reason to be bullied was that she had a "freak body".
In the original game it was much clearer, in her base 3d model she got what clearly looked like a male reproductive organ. On this occasion both her chest and this have been toned down but it's still there. I don't know what more proof you need.
These will always be controversial parts of Nier lore like the brother prostituted himself in seaside town but they are real and part of the novels and canon.

Yes she's a futanari, meaning she has both sexual organs ("She was of both sexes!"). It's due to a bug in the replication of her body, even if it's never stated if it happens everytime or if this Kainé is the first to get that...feature
It didn't happen because of the shade, she's born like that (that part is much clearer in the JP version when you have the description of her youth, the boys stripping her naked and laughing at her "freak" body).
But she considers herself as a woman, even with that little...addition. She's pretty insecure about it tho, hence why she wear overly sexualized outfit (that, and the fact that she wants to monitor the shade as revealed in Re[In]carnation)
About Emil, he went from being a clear dude to a ... thing. I think it transcend the concept of "transgender". He's just not human anymore. If one thing, it could be transhumanism
