Maybe you're confusing it with crew that goes into ships, i think they get reduced from your population count. I also was thinking how did i loose someone then i realized that its the crew of the ships. Different ships take different number of people as crew.
What i mean is, you start a new game, 80 crew, 1 minute later 79 crew left, and 1 death count in the data sheet. At this point we don't have ships or anything yet.
Yeah, but those options are from the Administrator's point of view. It even has ellipses after it ("they'll make it...") unlike any other science choice in the game. The admin believes they'll make it due to their exoskeletons, but ultimately they fail. I waited the whole day for them in the drone swarm, so sad.

Having adhequate medical facility would result in no death.
A no death run is not possible, as story event will always kill 5 of your science crew no matter the choice, all the rest are avoidable.
Accidents on extra hours are guaranteed. I literally had an accident pop up with time paused because i turned a building on that caused extra hours.
The only way to not loose anyone is by cheating with loading and reading about science expeditions outcomes.
