I'm playing in borderless mode, Vsync off and framerate set to auto. Perhaps try that and see if it helps with timing issues.
A little annoying but it works
I'm surprised that they never fixed it in the time since, though, since they did fix the fishing in Yakuza 3 which the higher framerate totally borked. Or at least I think they did, I haven't gone back to play Yakuza 3 since my first time through it.
For some odd reason, turning the refresh rate down to 30 still ran the game at 60fps and all the timing was perfect, until i closed the game. Now no matter what refresh rate or framerate settings i change the timing is still extremely off, which is really annoying when i know the song by heart and would very much be able to get a perfect score if it was on time.
