For reapers, you have chances to drop a new one everytime you close a breach (you find some breach in the campaign, you find them more in the battlefied and also in the slorm temple as each room is a breach, but you don't find them in the forge).
When you close a breach and you don't get a reaper, chances to drop on the next breach increase slightly.
On the 150th breach since your last reaper drop, you have a sure drop for a reaper.
Once you have all the reaper that can drop (some can also be obtained as a specific reward, some can be bought to a NPC and quite a few are evolutions), you can't drop any more reaper.
Once you have a reaper with a character, you have the possibility on another character to craft the equivalent reaper.
Ex: you get the Most Powerful Sword on the Knight, you can then craft the Most Powerful Bow for the Huntress and the Most Powerful Staff for the Mage.
They added more than half the reapers less than a month ago, changing how difficult it is to get a specific reaper. It's not impossible some changes happen by the 1.0.
When you get up to speed and start closing breaches left and right (especially on battlefield), it's not exactly that hard. I've had up to 3 of them drop over 10-20 floors of the battlefield when I closed every single breach I saw.
