The way that they're listed can be a bit confusing, since some of them come both separately and bundled together, but at the moment, what is there is

The GP-* DLC are basically expansion packs that add new modes and content. The Visual Pack adds some new clothes, accessories, tattoos, etc. along with a couple of rooms. The new maid personalities add the ability to hire a maid with that personality, and the GP-01 and GP-02 patches are required to make each new maid work with the GP-01 and GP-02 content (they work without it, just not with the new stuff that was added with GP-01 and GP-02). S-court has yet to release compatibility patches for GP-01 Fb. The store pages for each piece of larger DLC should have a link to a more in-depth description, but sometimes, they can be a bit hard to find in the mess of text. They would be

I think that we have most (all?) of the GP-* DLC that the Japanese version has, but we're _way_ behind on the additional maid personalities. They're coming but very slowly. I think that GP-02 requires GP-01, and GP-01 Fb may as well (I'm not sure), but the various maids don't depend on any particular DLC beyond the patches needed to make them work with the GP-* DLC. So, you can choose to buy it all or pick and choose.
To install the larger DLC, you have to download an installer for each one and run it to install that DLC.
In addition to the larger DLC, there is also a store for smaller DLC that the in-game menu will send you to. It can be found here: https://com3d2-shop-en.s-court.me/top.php?lo=0
This includes individual items and sets which combine several of the individual items. The individual items could be new yotogi (i.e. sex positions / scenes), new clothes, new hair, new rooms, new dances, new events, etc. It's the same kind of content found in the Visual Pack except that these are all split up, and s-court keeps putting out more every week (and sometimes a couple of times a week). So, you can pick a few items you want to buy, buy them all (presumably by getting the sets, since they cost less than buying everything individually), or whatever you feel like in between. Most of the smaller DLC are independent of one another (e.g. one outfit does not depend on another), but you probably do have to buy the "NTR Side" and "Pure Love Side" volumes in order, since they add events in those story lines.
To install the smaller DLC, you have to download it while the game is running, and then the game does the download instead of the browser, and the game then installs what it downloaded the next time you start it.
In addition to the larger DLC and smaller DLC, there is also a small number of DLC you can buy using "CR." You get some number of CR every time you buy any DLC from s-court, so they're basically loyalty points. The CR items can be gotten here: https://cr-en.s-court.me/
Similar to the larger DLC, to install the CR items, you have to download installers and run them to install the content.

Also, the launcher for the game does have a brief note indicating when new DLC or a new update gets posted, though it pretty much just says that they exist rather than giving any details.
For the game updates, you'll need to download them yourself and install them. They can be gotten here: http://com3d2.world/r18/update/
And if you want to remove the mosaics from the game, then you'll need to install some mods for that. A mod pack which is kept up-to-date (unlike the one listed in the guides here on steam) can be obtained here: https://github.com/krypto5863/COM-Modular-Installer
It includes a variety of mods. So, read the pdf that comes with it for instructions and select whichever mods you want when you run the installer. Some of them will not work with the international version of the game, but there is an option to just install the ones which are known to work with the international version, and it should detect which version you're running.
