Koikatsu has better customisation, especially with mods, and probably (although don't quote me on that) a better pose variety.
CM has management and character training aspects, variety of events ranging from girls doing stuff like watering plants, dusting and stuff, to girls being decieved, drugged and r@ped on a rooftop by 6 dudes, and in my opinion, a better story overall.
I'd definitely recommend playing both.
COM3D2 has one of best character creations ( second place ) and gameplay is awesome and hundred of H positions as default + more to add with mods
koikatsu has school simulator and with DLCs it gets even more open
COM3D2 has night club boss simulator but 30% of your time is on menus
Koikatsu has +20 personalities and really done great there
COM3D2 has 3-4 personality and gets repetitive in dialogues
i would pick COM3D2 instead of Koikatsu if i had money problem
Author even adds extra tease and sex poses and animations, though it will be a slow process because they are very difficult to implement

As for creation, I say it's 100 times better than com3d2, the devs building Vkatus made some crazy characters via accessories, more so from fans. Pretty awesome seeing a lot of neat tricks with all the accessories the players she/he wants. Even I would have never thought of something like that.
Really? I must have been missing something with CM3D2 then because it just seemed like a game with sex and nothing else, no real story or anything?
a lot of gamers on every game skip dialogues then complain the story is shet or it doesn't have story at all
COM3D2 has a lot of events with fully voiced dialogues
