Report a translation problem
The first part is easily fixed from Settings -> Controls. There's an option to override the displayed input type.
For the other issues, I'm surprised enabling Steam Input didn't work. Sometimes I wonder how Steam Input is even supposed to function 😂 Do you by chance have custom mappings for Steam Input or did you simply try enabling it? If the former, could you send a picture of the remapping?
Can you check you have the latest firmware? Sony has a tool for Windows to update it (https://controller.dl.playstation.net/controller/lang/en/2100004.html). After updating be sure to turn your controller off and back on again. I had a minor panic where after installing the update, I got no inputs from the controller.
As a quick sanity check, there's an online input checker ( https://hardwaretester.com/gamepad) where you can verify that the buttons are being recognized correctly.
Are you by chancing using any 3rd party controller tools or looked into using any like DS4Windows (https://ds4-windows.com/)? DS4Windows basically wraps the PlayStation inputs and converts them to XBox inputs.
