Hmm. Wait... what dungeon is that again, exactly? You're not talking about the steel key, are you? That's one of the more common bugs still left.
As a general rule, you can try activating the song "strugglers' lament"; it was a new song they introduced with the DC, patch 2 I believe, and it opens up most doors/gates. As a consequence, it also helps with these particular bugs where you get locked in, often.
Also, there's a "save anywhere" option now, be sure to save a lot. :-)
