The map is one big map, so I dunno if that counts as open-world to you. There is no loot outside of whatever dragons you kill can be free food if you even find the time to stop and eat their bodies.
It's not in the game yet, but a future plan is to add an inventory system and with it 'loot' in the form of materials to build nests and dragon pearls. Dragon Pearls are an item that will have between 1-3 buffs on them, buffs varying from stat boosts of varying degrees, to something as harmless as a cosmetic change like a pet following you or jellies floating over your head. These pearls can be traded on both PVP and PVE. On PVP, if you kill a player with a pearl equipped and you eat their body, you loot the first equipped pearl they had. Again, not in the game yet, but something to look forward to or wait for =)
As for PvE...there's not much in way of environmental danger yet. You can die if you don't know how to fight the scarabs and you can poison yourself but honestly I've done some messing around and it seems you almost have to TRY to poison yourself to actually die from the toxin. Eventually they want to add the elementals which will be a player faction that can attack you even on a PvE server. I think there's a good chance some dragons will have to watch out for lava in a specific future biome too. And I think nastier wildlife will be coming.
Back when this game was still an experiment using temporary store assets they had a freaky giant worm thing. I think there's suppose to eventually be a return of that just...as its own creature not with a borrowed model.
PVE only servers exist, but once you grow to flying age the game loses any sense of difficulty as you can easily get food and water. It basically becomes dragon breeding simulator for the stats.
