Being a Windows users helps that gives us some other options to run through for fixing that.
1-We should definitely look at running both steam and the game as administrator. Just in case its a permission issue. To do that we need to right click on the ootp21.exe icon. You'll find the icon by going to your steam library and right clicking the game then going to 'Properties' > 'Local Files' > 'Browse Local Files'. That will open the folder ootp is installed to and you'll see the ootp21.exe icon there. Right click that and then go to 'properties' > 'compatibility' you can select the option to 'run this program as administrator' on that screen and then every time you start the game it will automatically run in admin mode.
You'll also want to do the same thing for the Steam.exe program file itself as well. The steam.exe file one you can find most likely in C:\Program Files (x86)\Steam.
2-You may want to uninstall OOTP21 after doing that admin change and reinstall it with the new permissions. Q-Are you installing the game to a steam library on an external hard drive by any chance?
Let's start with those two and see if that makes a difference for you.

2-No, I'm using my main hard drive.
There is one thing I found strange, when the game is installed, there is no ootp folder in my documents
