Exclamation mark over Tav's head not going away !

Starting my first playthrough I landed on my head. Apparently so hard on the beach that my Tav now walks around with an exclamation mark floating over his head. Checked my journal and clicked all quests to make sure this wasn't some kind of notification but to no avail.

Anyone who have encountered something like this?
