Did anyone else not have the hag spawn in? Spoilers

I was going through the Auntie Ethel quest, got past everything in Ethels den, and at the end of it, in the room with the captured woman the Ethel didn't spawn in like she should have. I've tried re-loading the area, restarting the game. She wont spawn in, so I can't complete the quest.

Looks like i am encountering this also, was surprised when i came across the empty room when i was anticipating a boss fight for sure.

It's happened to me as well. I found a video on Youtube on how you can solve this by using an area of effect attack on a certain spot. Apparently she's invisible. The bug isn't that she isn't there. She doesn't become aggressive. When you attack her, she will become visible, but she will still not attack you.
