Issue with the Corvid Token in Act 3?

I purchased and equipped the Corvid Token in Act 3 and the Polymorph: Dire Raven class action is nowhere to be found. It does not appear in my spellbook or my hotbar. I have tried two other characters on this save file and am experiencing the same issue. Any help?
