Dark Urge run - Butler never showing up bug?

Need Help with Durge

Currently on a Durge run and I am in Act 2. The butler never showed up for me in Act 1, even after the bard scene. I have done multiple long rests but nothing. I continued and now I am in Act 2, after defending the last light inn, I get dialogue with Isobel about the urge wanted me to kill her, but I never actually get that scene and the butler does not show up, yet again. Now my quest is stuck on Get over it, Spill some blood. I resisted the urge by the way, not sure if that has any effect.

I don't know what to do now. Is my run ruined? Please help, I have put in over 40 hours of game play, I can't just reload or start a new gameplay.

If you are running any mods, they could be the cause. Otherwise do a clean reinstall. Follow the guides in this forum.
