Non-Lethal Knock Outs Are Being Counted As Deaths (Act III Spoilers)
During the Save Varna quest, when you go into the cellar of the Blushing Mermaid, there are four NPCs wearing masks. After knocking them out and freeing them from Auntie Ethel everyone in the Blushing Mermaid considers them as being killed.

Edit: I'm also assuming that the people in Auntie Ethel's lair in Act I are also considered dead even if you knocked them out by using non-lethal attacks.

It's breaking the game. I just knocked out Maryna's brothers, and when you get to Ethel house you have the option to say you just knocked them out, Maryna still react as they're all dead.
I wonder if it will break Wyll and Karlach first interaction.
Think, instead of killing Karlach, just knock her out, rest to get Wyll reward and recruit Karlach after... Is it possible? Or knock out Khagga to get Zevlor reward? I also knocked Gur, and Astarion consider him dead. Will I meet Gur later?

I'm not sure about the other things you mentioned, but Gandrel is in the beginning of ACT III. He has a very minor role. Although it does not matter, it's cool to see previous people alive and well. From the looks of it though, they're probably all considered dead and do not reappear if knocked out.

Afaik the masked warriors are the only exception.
Basicly the two brothers to be saved was already aksed in EA, but they never bothered with it. Also the whole non-lethal attack is just a pointless system. Which used up a lot of development time for not even being used:)

I haven't encountered those specific NPCs in Act 3, but knocking people out can definitely break certain things because I had a similar experience:

In Act 1, I rescued Halsin before going anywhere near Minthara, but rescuing him turns the entire goblin camp hostile. Oddly, when I came out of the prison area, the enemies in Minthara's room (including her) were marked as "temporarily hostile", which means you should be able to disengage or knock them out and then they'll be chill again after a long rest. So, I killed all of the goblins, but made sure to knock Minthara out just to see what would happen

Well, as it turns out, knocking her out completely bugs out her AI and, after a long rest, she just stood there doing absolutely nothing from that point forward. I could mouse over her and the cursor would turn into a speech bubble as if I could talk to her, but it didn't do anything. Even attacking her had very inconsistent behavior because she turned hostile the first time I did it, but reloading and attacking her against caused her to completely ignore any attacks from then on. I moved on to Act 2, wondering if she'd re-appear at Moonrise (since I knew she'd make an appearance there from pre-release footage), but she was completely absent from the entire thing, as if the game just assumed she was dead

She's probably still there in the goblin camp, completely nude with nothing to her name except an un-lootable mace, just blankly staring at the corpses littered around the room
