Auntie Ethel's Well - Permanent HP loss
I don't know if they patched something or what, but after killing her, I drank from Auntie Ethel's well before some rather tough fights in the Underdark for the HP buff, then, the next time I rested, I lost both the HP buff from the well (expected) and another 10-12 HP on every character. Pretty sure that's NOT supposed to happen, but now I have to repeat several hours of gameplay to fix this.

You get a consequence from drinking from that well - but AFAIK it isn't permanent. I got rid of it after yet another long rest.

During the fight with the hag this buff is also turned into a debuff (just adding this for completeness)

Yes, you get the "Nauseated" debuff - but that debuff doesn't reduce HP. I also rested a second time to remove that debuff and still had the HP loss.

Mmh.. I'll try to reproduce it next time I meet that hag. Your exact sequence was like this:

- Killed hag - Goto well - Drank from well -> buff - Goto underdark - Goto camp - Rested -> buff gone, permanent HP reduction

Play a lot
Decide I want a HP buff in the Underdark for some of the nastier fights
Rest up after killing a ton of stuff for hours without a long rest
Seemingly permanent HP reduction

Don't mind the 0h0m notation - I'm on a secondary PC today - issue happens on both, with or without mods.

I just double-checked and took 3 full rests. 1 that triggered the HP loss, and 2 more. Still lost like 20-22 HP (instead of 10). I also tried doing a 'reset' with Withers, just in case, but had the same issue.

I double-checked an old save and found that the first time I used it, there was no permanent HP loss afterwards.
I tried sleeping somewhere else as well - just in case. No change.

I checked several variations now (killed hag, let hag live) and so far I can't reproduce your permanent debuff. Details:

Letting Hag live: - status well: dead flesh lies below - drank water - +10 HP buff - sleep at camp OR doing some fights elsewhere (underdark, gith patrol) -> no difference - variation A: buff ends - variation B: buff ends, Nauseated debuff (seems to be applied randomly, is removed after another rest)

I also had another variation where one of my characters died while having the +10 HP buff and I had to resurrect (using Wither). Still the same result, no permanent debuff for me.

All in all a pretty consistent behaviour for me. Odd. My char is a paladin (oath of the ancient) - maybe he shrugs off any permanent debuff. ¯\_(ツ)_/¯

Looks like each character had lost exactly 13 health permanently - such a weird number to permanently lose. I def won't be drinking from that well again.
