Killer's Sweetheart ring
It seems this ring does not drop normally in the trial. Shar's trial mirror fight

It only seems to drop if you let your main character be disarmed by one of the mirror warriors(likely the gith mirror). Once that happens and you finish off your own mirror (with ranged most likely), then the ring drops.
If you are not disarmed during the fight the ring does not seem to drop. I have done the fight 2 times, and the only times i got the item was when my main got disarmed. (By getting accidentally disarmed in one of the fights is how i noticed it, was like my second try.)
Once i noticed this I started to test it out, and it seems only by being hit by disarm and SUCCESSFULLY DROPPING your weapons will this ring spawn.
If the disarm does not succeed, miss, or if you drop your weapons on your own it will not spawn.

For anyone who stumbles across this and is confused as to why they are not getting the ring, I now know why after experimenting a little. I too was also not getting the ring, but then I considered the name: Killers Sweetheart

To get this ring, your "Sweetheart" must be in your group, which the game takes as the companion you have the highest affection level with (I don't have a romance partner in the playthrough I tested this on, but I did get this ring to drop with Minthara, whom I have the highest affection with).

Your main player character must then deliver the final killing blow to your "Sweetheart", directly or indirectly with aoe concentration spells. The ring will then successfully drop.

Sadly no. If you kill another character's mirror, or even attack you get a stacking "curse" that reduces ability scores.
I have done testings as well, because of this. The ring CAN drop what you describe, but it is not 100% it will drop.
However if you let your main character be disarmed during the fight, it will drop a 100%.

edit: i was not really clear that i've done the fight multiple(about 20+) times, and i tested out this scenario regarding the disarm event happening 2 additional times after i noticed the ring drop trigger. I've done it because i did not want to get the curse. And even a reaction attack could give it to you.

edit2: unless patch 1 hotfix 5 changed things.

I got the ring by solo'ing the trial with Shadowheart (who was the MC's LI) . No disarming or MC involved in the fight

Requiring a disarm would definitely screw over my main character, a monk. He has nothing to disarm.
