She now sits in my camp, gear and all, with an option to add her to the party which doesn’t actually do anything.
The journal entry said she left. I’m a little bummed but not enough to run a trainer
You need to avoid getting close to Dame as that will trigger the cutscene immediately, instead go around her to the door where Isobel is waiting. Go behind her and talk to her, the same cutscene will play. You can invite them to your camp if you want, and then immediately after you finish the conversation, do a long rest (try to be quick). For me, doing it this way did not make Jaheira disapprove and so she stayed in my party. I also made sure to talk to Jaheira before doing this and also when I got back to camp, although I think that doesn't matter.
It seems that if Isobel steps on some damaging ground during or after the cutscene, it will make Jaheira disapprove, and in my case there was such ground near the doors. Not sure if this was fixed in a later patch.
It would be one thing if the bug just made her permanently leave and STAY AWAY, but she just magically appeared back in camp, I can talk to her about my deeds outside the camp and stuff, which makes it even more annoying.
The bug makes the game think she both left and is still a companion at the same time. I tried to look for some config file to possibly edit companion approvals, but I couldn't find anything.
Too late for me tho, I should of tried it back then but now I'm well into act 3 and i would loose 10ish hours of gameplay.
